In-Home Assessments

Non-medical Assessments are conducted to find clients' functioning capabilities and identify their needs. Nursing Assessments are an expanded version of the Non-medical Assessment as they more thoroughly examine and identify health conditions.

In both situations, assessors may review subsidization possibilities, develop care plans, make referrals, and arrange for service delivery schedules according to the your needs.

Nursing Services

Our community nurses provide care to people of all ages.

They develop customized care plans and work with the clients, their families, and their caregivers to help them remain at home.

Our nurses can facilitate a seamless transition from hospital to homecare.

They provide in-home treatment, care support and/or education

Health System Navigation & Case Management

At MedEx we understand that transitioning from hospital to home and navigating the health system can be an overwhelming process.

Our Case Managers arrange and manage all your health service needs, they:

• Coordinate services and access community resources.
• Support in navigating the health care system
• In-home consultation and completing initial and ongoing assessments
• Provide tailored information
